An AI-generated illustration should not be relied on for precise clinical detail or used as a substitute for a clinician’s explanation. Forms and interactive features
We aim to make contact and appointment forms understandable and operable by keyboard and assistive technology. Some patient forms, portals, maps, scheduling services, videos, chat features, or accessibility tools may be provided by third parties and may present barriers that we do not control directly. If you cannot complete a form, read a document, use an interactive feature, or reach a linked service, call us. We can help you obtain the information or take the next step through an alternative channel appropriate to your needs. Do not send urgent medical concerns through an accessibility feedback form.
