Your Privacy

BioHealth Pain Management (“BioHealth,” “we,” “our,” or “us”) respects your privacy. This Privacy Policy describes information collected through this public Website, how it may be used, the parties that may help operate Website features, and choices available to you. It applies to website visitors and online inquiries. It does not replace the practice’s separate HIPAA Notice of Privacy Practices, which governs protected health information handled in connection with patient care, payment, and health care operations. Where health information submitted online is protected by HIPAA or another applicable law, those requirements continue to apply.

1. Information you provide

Depending on the features you use, we may receive your name, email address, phone number, preferred location or clinician, appointment request details, the text of your message, and any files or other information you choose to submit. We may also receive information you provide when requesting a callback, subscribing to updates, responding to a survey, sending feedback, or interacting with an approved patient service. If an optional text-message checkbox appears, we may record your phone number, consent selection, and related message history to administer that program.

Please limit ordinary contact forms and email to the information needed for your request. The current general contact form states that information is forwarded to the office by email rather than a secure messaging system. Do not submit detailed medical history, identification numbers, payment card information, or records through a general form. To discuss confidential information, call 424-360-0066 for instructions on a suitable channel. If a particular patient form or portal is designated as secure, read the information displayed there and its applicable privacy notice.

2. Information collected automatically

When you visit the Website, the site and its service providers may collect technical and usage information such as IP address, approximate location inferred from IP address, browser and device type, operating system, referring page, pages viewed, links clicked, visit times, and error or security logs. Cookies and similar technologies may support basic site operation, remember preferences, help understand traffic, prevent abuse, or measure the performance of Website content. The precise tools used can change as the Website is updated. Browser settings and, where offered, Website cookie controls may let you restrict nonessential cookies; some functions may work differently if you do.

Visits to health-related pages, appointment interactions, form fields, and portal pages can raise special privacy concerns. BioHealth should assess any analytics, advertising pixels, embedded content, chat tools, and form integrations before deployment and configure them consistently with applicable health privacy requirements. This policy alone does not authorize disclosure of protected health information to a tracking vendor.

3. How we use information

We may use information to operate and secure the Website; respond to inquiries; route appointment requests; provide information you request; administer consented communications; improve navigation, content, and accessibility; identify technical problems or misuse; comply with legal obligations; and protect patients, visitors, and the practice. We use patient information for treatment, payment, and health care operations as described in the practice’s HIPAA Notice of Privacy Practices and applicable law. We do not treat a visit to a condition page as permission to disclose sensitive health information for advertising.

4. Forms, email, SMS, and patient services

Online submissions may be reviewed by authorized practice staff and, where necessary, service providers that deliver or support them. Ordinary email and standard text messages may be less secure than an approved patient portal. Do not rely on these channels for emergencies. If you opt in to practice text messages, message frequency may vary and message and data rates may apply. Reply STOP to opt out, HELP for assistance, or call 424-360-0066. We do not sell or share your mobile number or text-message opt-in information with third parties for their own marketing or promotional purposes. A messaging provider may process information as needed to deliver messages on our behalf.

A linked patient portal, scheduling platform, intake form, or payment service may collect information under its own notice. The presence of a link does not mean its data practices are identical to those of this public Website. For questions about access to medical records or how the practice uses protected health information, refer to the HIPAA Notice of Privacy Practices and contact the office.

5. AI-assisted content and interactive tools

The Website may contain text or visual assets prepared with AI assistance and reviewed for publication. Merely viewing such content does not mean your personal information is being sent to a generative AI service. If BioHealth later offers an AI-powered chat or interactive assistant, the feature should display a notice explaining the provider, information it collects, whether messages are retained or reviewed, and whether it is an approved channel for health information. Unless the feature expressly says it is a secure patient channel, do not enter diagnoses, records, account credentials, or other sensitive information. We do not represent that an automated response is medical advice.

6. When information may be shared

Information may be shared with authorized staff and vendors that help host, maintain, secure, analyze, or operate the Website; deliver forms or communications; provide requested scheduling or patient services; or assist with legal compliance. We may also disclose information when required or permitted by law, to protect rights or safety, or with your direction or authorization. Vendors that handle protected health information on our behalf must be evaluated for applicable privacy and security obligations, including a business associate agreement when required. We do not sell patient medical information. We do not sell mobile numbers or SMS opt-in information for third-party marketing. Any use of advertising or cross-site tracking must be reviewed against the actual deployment and applicable law; this policy is not consent or authorization for an otherwise prohibited disclosure.

7. Images, videos, testimonials, and public posts

Images on the Website may be licensed stock photography, illustrations, AI-assisted imagery, or practice photographs. A person shown in a generic image is not necessarily a patient or clinician. Patient photos, stories, testimonials, and before-and-after images should be published only with the permissions required by applicable law and practice policy. If you post publicly on a review site or social platform, that platform controls the visibility of your post. Avoid sharing private medical information in public comments; contact the office through an appropriate private channel instead.

8. Security and retention

We use administrative, technical, and organizational measures appropriate to the information and systems involved. No Internet transmission or storage method is completely risk-free. We retain Website information for the period reasonably needed for the purposes described above, operational records, legal obligations, dispute resolution, or security, then delete or de-identify it as appropriate. Medical records and protected health information may be subject to separate retention rules and the practice’s patient privacy procedures.

9. Your choices and privacy requests

You may decline to provide optional information, adjust browser cookie settings, and opt out of a text program as described above. You may contact us to ask about website information you submitted, request a correction, or ask that information be deleted when applicable law permits. We may need to verify your identity and may retain information where law or legitimate recordkeeping requires it. Rights regarding protected health information, including access and amendment rights, are described in the HIPAA Notice of Privacy Practices. California residents may have additional rights for personal information covered by California privacy law; the availability of a particular right depends on the type of information and whether that law applies to the practice and information at issue. We will handle qualifying requests under applicable law and will not unlawfully discriminate for exercising a privacy right.

10. Children and external services

The Website is intended as general practice information and is not directed to children under 13. If you believe a child provided personal information through the Website without appropriate authorization, contact us. Links, videos, maps, social media embeds, and other third-party features may collect data under their own policies; review those policies before using them.

11. Updates and contact
