What to Bring
- Arrive a half hour early prior to your scheduled appointment time.
- Bring all test results and films with you at the time of your visit.
- Bring all original medication bottles.
- Bring all completed forms to avoid delay in filling out paperwork at the time of your visit.
- Bring your insurance cards and a photo ID.
