Welcome to the website of BioHealth Pain Management (“BioHealth,” “we,” “our,” or “us”). These Terms and Conditions govern your use of this website and its pages, articles, images, videos, forms, interactive features, and other online content (collectively, the “Website”). By using the Website, you agree to these Terms. If you do not agree, please stop using the Website. Additional terms may apply to a patient portal, appointment platform, telehealth service, or other third-party service reached through the Website.

1. Information only; no medical advice

Website content is provided for general educational purposes. It is not a diagnosis, treatment recommendation, medical opinion about your individual circumstances, or a substitute for an in-person or other appropriate evaluation by a qualified clinician. Descriptions of conditions, procedures, expected results, recovery, or candidacy are general and may not apply to you. Treatment choices, risks, benefits, alternatives, and insurance coverage depend on your particular situation and should be discussed with your treating clinician.

Reading the Website, contacting the practice, submitting a form, or using an online tool does not by itself establish a clinician-patient relationship. An appointment request is not confirmed until the practice confirms it. Do not delay seeking medical care because of something you read here.

The Website may offer contact, appointment request, referral, feedback, or other forms. Provide accurate information and use the form only for its stated purpose. A general contact form may be delivered to the office by email and may not be a secure channel for detailed health information. Unless a form specifically identifies itself as an approved secure patient channel, do not enter a Social Security number, payment card number, medical record, detailed symptoms, diagnoses, or other sensitive information. If you need to share clinical records or discuss confidential medical matters, call the office at 424-360-0066 and ask for an appropriate secure method.

Submitting a form does not guarantee receipt, an appointment, a response within a particular time, or an offer of care. We may contact you using the information you provide to respond to your request. Electronic communications can be delayed, misdirected, or intercepted despite reasonable precautions. Where the Website offers an optional SMS consent checkbox, text messages are governed by the disclosures displayed with that checkbox and any applicable SMS terms. Message frequency may vary; message and data rates may apply. Reply STOP to opt out of a text program and HELP for assistance, or call 424-360-0066. Consent to promotional texts, if any are offered, is separate from receiving medical care. Do not send urgent or sensitive medical details by ordinary text unless the practice has directed you to an approved secure channel. 4. AI-assisted content and automated features

Some Website copy, illustrations, images, accessibility aids, search features, or other content may be created or improved with artificial intelligence (AI) and reviewed before publication. AI-assisted material may be incomplete, outdated, or inaccurate. An image or illustration may be representative rather than a photograph of an actual patient, provider, procedure, facility, device, or treatment result. If an automated chat, symptom guide, or similar interactive tool is made available, it may generate responses without a clinician reviewing each answer. It cannot diagnose, prescribe, triage an emergency, establish a clinician-patient relationship, or replace professional medical advice. Follow any tool-specific notice about whether a conversation is stored or reviewed, and do not enter sensitive health information unless the tool expressly states that it is an approved secure channel. 5. Ownership and permitted use

Unless otherwise stated, the Website’s text, design, graphics, photographs, illustrations, videos, logos, and other materials are owned by BioHealth or used with permission and are protected by applicable intellectual property laws. You may view and print a reasonable amount of content for your personal, noncommercial use, provided you retain notices and do not misrepresent its source. You may not reproduce, republish, sell, scrape, systematically collect, train an AI system on, or create derivative works from Website content without prior written permission, except as allowed by law. Sending feedback does not authorize us to use a patient’s name, image, health information, or testimonial for marketing where separate permission is required.

You agree not to interfere with the Website, attempt unauthorized access, introduce malicious code, misuse forms, impersonate another person, or use the Website for unlawful purposes. We may change, suspend, or discontinue Website features and correct errors without notice. We work to keep information current, but services, clinicians, locations, hours, insurance participation, pricing, and availability may change; please confirm material details with the office.

9. Disclaimers and limits

The Website is provided on an “as available” basis. To the extent permitted by applicable law, BioHealth does not promise uninterrupted operation, error-free content, or that online information will meet every person’s needs. Nothing in these Terms limits rights that cannot lawfully be limited, waives duties owed in the provision of professional medical care, or changes rights under applicable privacy law. Any limitation of liability for Website use applies only to the extent permitted by law and does not apply where prohibited.
